JOB TITLE: Qualified Mental Health Professional- (QMHP-C or QMHP-A)

DUTIES AND RESPONSIBILITIES:

  • Provide one-on-one behavioral support services to clients experiencing a crisis jeopardizing their current placement.
  • Initiate and facilitate de-escalation techniques and strategies to assist the client in maintaining self-control to prevent hospitalization and/or re-hospitalization.
  • Use good interpersonal communication skills to gain client cooperation and resolve conflicts.
  • Interact with all clients in accordance with the individual service plan and the therapeutic process.
  • Exercise appropriate intervention in accordance with the least restrictive alternative to protect client from harming self or others.
  • Ensure that every Incident Report is completed immediately after the incident and submitted to the direct supervisor or Director the same day
  • Be available for 24 Hour Emergency Crisis Response to assigned participant
  • Develop and implement plans for discharge including mobilization of resources for ongoing maintenance, rehabilitation & recovery.
  • Develop assessments, individualized service plans, progress notes, and discharge summaries.
  • Provide support and linkage to community resources for participants
  • Provide progress notes for each meeting with clients to document progress or barriers toward reaching goals and objectives of client’s service plan.
  • Complete all Progress Notes, Quarterly Reports, Individual Service Plans and Discharge Summaries on or before the due date
  • Other duties as assigned

IMMEDIATE SUPERVISOR: Clinical Supervisor

MINIMUM QUALIFICATIONS: (I) A Bachelor’s Degree or Master’s Degree in Social Work from accredited college or university with minimum of 1 year of documented clinical experience with children & adolescents and/or adults. (II) Be a doctor of medicine or osteopathy licensed in Virginia. (III) Have a master’s degree in psychology from an accredited college or university with least one year of clinical experience with children & adolescents and/or adults. (IV) Be a registered nurse with at least one year of clinical experience with children & adolescents and/or adults. (V) Have at least a bachelor’s degree in a human services field or in special education from an accredited college with at least one year of clinical experience with children and adolescents. (VI) Be a licensed mental health professional. Must possess a valid driver’s license. Experience in providing psychiatric or mental health services to children and or adults who have a mental illness. Must possess strong listening and interviewing skills; as well as basic computer/typing skills. Strong verbal and written communication skills. Certification in CPR- infant, child, and adult. TB test required. Must also have reliable transportation and car insurance
